Data-driven governance for global change: the National Health Prevention Hub (NHPH) as a science–policy interface for cancer prevention

Background
Cancer prevention is the most effective strategy for reducing disease incidence and ensuring the long-term sustainability of healthcare systems. However, its impact is often limited by fragmented screening programs, persistent data silos, and insufficient integration of human, environmental, and societal determinants of health. Within the framework of the Italian National Recovery and Resilience Plan (PNRR), the Ministry of Health has developed the National Health Prevention Hub (NHPH). This strategic initiative acts as a pioneering science–policy interface, designed to bridge the gap between cutting-edge oncological research and regional public health implementation, consolidating disparate efforts into a cohesive, One Health–oriented national strategy.
Objectives
The NHPH aims to establish a robust digital ecosystem capable of transforming scientific evidence into actionable health policies, thereby strengthening health system resilience. Its core objectives include harmonizing national prevention programs to reduce regional disparities and ensure equitable access to precision diagnostics. By leveraging artificial intelligence and advanced analytics, the Hub seeks to anticipate the needs of high-risk populations while addressing broader environmental and commercial determinants of health. Ultimately, the NHPH operationalizes a One Health approach by translating integrated health and environmental data into concrete cancer and chronic disease prevention actions.
Methods
The NHPH is conceived as a multisectoral interoperability hub rather than a simple data repository. It adopts an integrated approach to connect pre-existing, non-interoperable platforms, including cancer registries, electronic health records, and environmental exposure databases. Its technological core is a centralized data lake that integrates clinical, genomic, socio-economic, and environmental datasets. Through AI-supported analytical tools and standardized data models, the Hub enables population risk stratification and long-term trend simulation. This framework supports interdisciplinary collaboration among epidemiologists, oncologists, and policy-makers, ensuring alignment with European data standards while maintaining a secure and ethical environment for large-scale analysis.
Results
Scheduled to reach full operational capacity by the second quarter of 2026, the NHPH replaces traditional, fragmented surveillance systems with a real-time, integrated monitoring infrastructure. By breaking down data silos, the Hub enables more precise risk assessments, including the evaluation of environmental carcinogens and lifestyle factors, as well as rigorous cost-effectiveness analyses of innovative screening protocols. Preliminary results indicate improved institutional coordination and enhanced capacity for anticipatory risk assessment, allowing financial resources to be dynamically allocated to the most impactful primary and secondary prevention interventions.
Implications
The National Health Prevention Hub represents a research-driven shift from reactive healthcare toward a proactive, digitally enabled governance model. By institutionalizing a permanent science–policy interface, it strengthens the National Health Service’s ability to respond to global challenges and the growing burden of oncological diseases. This model offers a scalable benchmark for public health innovation, demonstrating how the integration of digital transformation and One Health principles can support equitable, sustainable, and evidence-based cancer prevention strategies worldwide.
